Privacy of Personal Health Information

This notice describes how Renova Dental collects, uses, discloses, and protects your personal health information as a patient of our dental practice. It is provided under Ontario’s Personal Health Information Protection Act, 2004 (PHIPA), together with Canada’s PIPEDA.

Our Role

As a dental practice in Ontario, Renova Dental is a “health information custodian” under PHIPA. We are responsible for the personal health information in our custody and control, and we take that responsibility seriously.

What Is Personal Health Information?

Personal health information (“PHI”) is identifying information about you that relates to your physical or oral health, the care you receive, your health history and that of your family, payments or eligibility for care, and similar information. Examples include your dental and medical history, examination and treatment records, x-rays and images, and insurance and billing details.

How We Collect PHI

We collect PHI directly from you whenever possible — for example, on intake forms and during appointments — and, with your consent, from other health-care providers, your insurer, or a government program such as the Canadian Dental Care Plan.

How We Use and Disclose PHI

We use and disclose your PHI to:

  • Provide, plan, and coordinate your dental care;
  • Communicate with other health-care providers involved in your care (your “circle of care”);
  • Process payment, insurance, and program claims;
  • Administer and manage our practice, including quality improvement; and
  • Comply with legal and regulatory requirements, including those of the Royal College of Dental Surgeons of Ontario (RCDSO).

Consent

Within your circle of care, we generally rely on your implied consent to use and disclose PHI for the purpose of providing you with dental care. For uses or disclosures outside of providing care, we will obtain your express consent unless the law permits or requires otherwise. You may withhold or withdraw your consent, subject to legal limits; we will explain the consequences of doing so where relevant.

Your Rights Under PHIPA

  • Access a copy of your records of PHI, subject to limited exceptions;
  • Request a correction if you believe your records are inaccurate or incomplete;
  • Withdraw or withhold consent to certain uses or disclosures; and
  • Be informed about how your PHI is handled and ask questions of our privacy contact.

Safeguards

We protect PHI with administrative, technical, and physical safeguards — including access controls, staff confidentiality obligations, and secure storage — appropriate to the sensitivity of the information.

Retention and Secure Destruction

We retain dental records for the period required by the RCDSO and applicable law, and we securely destroy PHI when it is no longer required. (Ontario dental record-retention requirements are generally measured in years from the date of last contact, with longer periods for records of minors; the exact period should be confirmed by the practice.)

Website Analytics Are Not Part of Your Health Record

Information collected automatically when you browse renovadental.ca (such as analytics and advertising data) is not part of your clinical record and is handled under our Privacy Policy. Please do not send personal health information through website forms or email, which are not secure channels; contact the office directly to discuss your care.

Questions and Complaints

Please direct any questions or concerns to our privacy contact using the details below. Renova Dental should designate a Privacy Contact Person / Health Information Custodian responsible for PHIPA compliance. If we cannot resolve your concern, you may contact the Information and Privacy Commissioner of Ontario (IPC): ipc.on.ca, toll-free 1-800-387-0073.
